Here, we’ve made your new patient paperwork available for you to download and complete at home, before your visit. On the Financial Agreement Form, you’ll find information about the payments we accept, including third-party financing through CareCredit and Lending Club.
At the first appointment, we’ll review your medical and dental history and enter your information in our system to establish you as a patient of our practice.
If you have trouble printing the forms, just let us know. We’ll be happy to email or mail them to you.
